What Scar Tissue Actually Is

Every scar, whatever its cause, is made of the same basic material: collagen fibres laid down quickly by the body to close a wound. It is not a replica of the tissue it replaces. Healthy Fascia is organised, laid down along the lines of stress and movement that the tissue experiences every day, with a structure that allows it to glide, stretch, and transmit force efficiently. Scar tissue is laid down fast and in a much more disorganised weave, dense, less elastic, and often bound tightly to the structures immediately beneath it rather than gliding freely over them.

That difference in structure is what makes scar tissue behave so differently from the surrounding Fascia. It does not move the way healthy tissue moves. It can adhere to muscle, to other layers of Fascia, or to nearby organs and structures, creating a fixed point in a system that is otherwise designed to move and distribute force continuously. Whether the scar came from planned surgery, an accident, or repeated smaller trauma, the underlying tissue behaves in the same restricted way.

How Scar Tissue and Fascial Adhesion Form

When Fascia is cut or torn, whether by a surgical incision or an injury, the body's priority is to close the gap as quickly as possible rather than to rebuild the original architecture. Collagen fibres are laid down in a dense, criss-crossed mesh rather than the aligned, layered structure of healthy Fascia. This new tissue is strong, which is exactly what the body needs in the days after an injury, but it is also stiffer and less able to glide against neighbouring structures.

Adhesion forms when this new tissue binds to the layer beneath it rather than remaining separate and mobile. This is a normal part of healing in the short term, but in many cases the binding does not release once healing is complete. The scar becomes fixed to the muscle, Fascia, or structure beneath it, and that fixed point changes how force travels through the surrounding tissue every time you move.

Repeated trauma produces a similar effect over time even without a single defining injury. Small, repeated strains or minor tears in the same area of Fascia can accumulate disorganised collagen in the same way a single larger injury does, producing an area of adhesion that developed gradually rather than in one event. The tissue itself, once formed, behaves the same way regardless of how it got there.

Research into Fascial healing has consistently shown that scar tissue and adhesion alter the mechanical properties of the surrounding system, restricting glide between tissue layers and increasing the density of pain receptors in the affected area. This is one of the more measurable and well-established findings in the modern understanding of Fascia, and it explains why a scar that looks fully healed on the surface can remain a substantial source of pain and restriction beneath it.

This depth of understanding of scar tissue is not part of standard surgical or physiotherapy training, which focuses on wound closure and surface healing rather than the long-term mechanical behaviour of the Fascia beneath. What Scar Tissue Does to Movement and Sensation

A fixed point of adhesion changes how the surrounding Fascial chain moves as a whole. Because Fascia transmits force continuously through connected lines, a restriction at one point creates compensation elsewhere, sometimes at a considerable distance from the scar itself. An abdominal scar can restrict rotation through the trunk and contribute to lower back pain. A knee scar can alter loading through the hip. A shoulder scar can limit the full overhead reach long after the incision itself has healed. Sensation changes too. The dense collagen and altered nerve supply within scar tissue often produce pulling, tightness, tingling, or a dull ache that patients frequently describe as different in character to their original pain, sometimes appearing months or years after the scar itself has stopped being visibly sensitive.

A scar that looks settled and a body that still feels restricted around it are not in conflict. The visible healing and the deep Fascial mobility are two separate processes, and the second one is often the part that has not been addressed. That restriction is directly treatable.

Common Questions

Yes. A scar can look completely settled on the surface while the Fascial tissue underneath it is still densely bound down to the layers beneath. Surface healing and deep Fascial mobility are two different processes, and the second one does not always resolve on its own. Pulling, tightness, or referred pain some distance from a well-healed scar is a common and directly treatable presentation.

Yes. Fascial adhesion does not have an expiry date. We regularly assess and treat scar tissue from surgery or injury that happened decades earlier, and the tissue responds in exactly the same way it would if the scar were recent. The restriction is about the quality and mobility of the tissue, not how long ago it formed.
