If you have been told you have tennis elbow and your honest reaction was that you have never picked up a racket in your life, you are not the exception. You are the majority. The lateral epicondyle does not know the difference between a poorly timed backhand and eight hours a day with a wrist kept in slight extension over a mouse. Both load the common extensor tendon the same way, repeatedly, over months or years.
Desk workers are among the most common tennis elbow presentations we see in Bath. The history is almost always the same: a job that involves sustained keyboard and mouse use, a slow build-up of discomfort on the outside of the elbow, and a diagnosis that sounds like it belongs to someone else's sport. It belongs to you as much as it belongs to any tennis player, and it responds to the same assessment.
A mouse used for hours keeps the wrist extensors under a low, constant, isometric load, which is a very different demand to the brief, powerful contractions those same muscles are built for. Low-level sustained tension of this kind is one of the most effective ways to develop a Fascial restriction, because the tissue never gets the chance to fully lengthen and recover between uses.
Typing adds a second layer. The fingers and wrist extensors work in short, repeated bursts thousands of times a day, and the shoulders are usually carried slightly forward and elevated for hours at a time. Restriction accumulates at the shoulder and through the extensor compartment simultaneously, and both feed load down to the same point at the lateral epicondyle. The same desk posture is also behind much of the wrist pain we see from typing, since it is the same Fascial chain being loaded, just further along it.
A proper desk assessment, the right chair height, a vertical mouse, a keyboard at the correct angle, reduces the strain going forward in a real way. It is good advice and worth having. What it cannot do is release a Fascial restriction that has already built up in the shoulder and forearm over the months or years before the change was made.
This is why so many desk workers make every recommended adjustment and still carry the same elbow pain weeks later. The setup stopped the problem getting worse. It did not touch the restriction that was already there, sitting in the arm chain, still loading the tendon exactly as before.

The assessment follows the Superficial Back Arm Line from the shoulder blade through the triceps and extensor compartment to the hand, the same line involved in any tennis elbow presentation, sporting or not. For desk workers we pay particular attention to the neck and shoulder, since a forward head position sustained for hours a day changes tension through this line before the mouse hand ever comes into it.
This dissection shows the Fascial continuity through the arm, the structure being assessed whether the loading came from sport or from a keyboard.
Once the shoulder and extensor compartment restriction is released, most desk workers notice a measurable reduction in lateral elbow pain within the first session, and a marked improvement in how the arm feels by the end of a working day. Combined with the ergonomic changes you have already made, releasing the underlying restriction gives the tendon a genuine chance to settle rather than staying permanently overloaded at a lower level.
Desk workers who have already tried an ergonomic overhaul and made no progress are often the quickest to notice change, because the missing piece has been sitting in the arm chain, not the desk.

Yes, and it is extremely common. Tennis elbow is a clinical description of where the pain is, not of how you got it. Anyone whose forearm extensors are loaded repeatedly, including keyboard and mouse work sustained for hours a day, can develop the same lateral epicondyle overload as a tennis player. We see far more desk workers with this presentation than we see tennis players.
Ergonomic changes reduce further strain going forward, which matters, but they do not release a Fascial restriction that has already developed in the shoulder, forearm, or extensor compartment from months or years of the previous position. The desk change stops the situation getting worse. It does not undo what has already built up in the arm chain, which is why the pain can persist even after a proper ergonomic review.

Physiotherapy assesses and treats the muscles and joints at the site of pain. It is skilled work and truly helps many presentations. What it does not assess is the Fascial system connecting those muscles and joints to the rest of the body.
