What Wrist Tendonitis Is Described As

Wrist tendonitis is the diagnosis given when one or more of the tendons crossing the wrist become inflamed, usually from repeated overuse beyond what the tissue can currently tolerate. When the inflamed tendons are specifically the two running through the thumb-side sheath, the diagnosis is usually named more precisely as De Quervain's tenosynovitis, a specific form of the same underlying problem. The examination that leads to this diagnosis is direct: tenderness directly over the affected tendon, pain that increases with resisted movement, sometimes visible swelling or a faint grating sensation when the tendon is asked to move under load.

Rest, anti-inflammatory medication, and sometimes a steroid injection are the standard response, and for many people they truly help in the short term. The inflammation settles. The pain reduces. For a period, everything seems resolved. Then normal activity resumes and, for a large number of patients, the same pain returns in the same place within weeks or months.

Why That Description Is Real, and Incomplete

Nothing about the tendonitis diagnosis is wrong. The tendon is inflamed. That finding is accurate and the treatment aimed at it, rest and reduced load, makes physiological sense as a first response. Where the picture runs out is in explaining why that particular tendon, in that particular wrist, became overloaded in the first place, and why the same overload tends to return once the tendon is asked to work normally again.

A tendon becomes inflamed when it is asked to manage more tension than it can currently absorb. Examining the tendon in isolation shows the inflammation. It does not show where that tension was generated. That question sits further up the chain, in the tissue connecting the tendon to the rest of the arm.

Fascia surrounds every tendon in the wrist and continues, uninterrupted, through the forearm, across the elbow, and into the shoulder and neck. When Fascia anywhere along that chain becomes restricted, it changes how force is distributed to the tendons at the wrist, concentrating load on structures that were never designed to carry it alone. This is the mechanism most standard tendonitis examinations are not built to find, because the examination is focused on the tendon rather than the chain feeding it.

What Is Feeding the Inflammation: The Arm Chain

The Anatomy Trains framework maps the tendons of the wrist as the far end of continuous Arm Lines running from the chest, upper back, and shoulder through the elbow and forearm. Restriction anywhere along those lines increases the tension arriving at the wrist tendons before a single repetitive movement has even begun. A tendon working through a restricted chain is starting every contraction already closer to its limit.

This explains a sequence many patients recognise once it is described to them: the tendonitis calms with rest and treatment, then reappears in the same spot within a similar timeframe each time normal activity resumes. The tendon is not failing to heal. It is healing into the same restricted chain that overloaded it the first time, because nothing above the wrist has changed.

Common Questions

A steroid injection reduces inflammation directly at the tendon, and for many people that brings real, welcome relief. What it does not do is change the load arriving at that tendon from the forearm, elbow, and shoulder above it. If that load has not changed, the tendon returns to the same conditions that irritated it in the first place once the injection wears off. This is one of the most common reasons wrist tendonitis returns after an injection that initially worked well.

They describe overlapping but slightly different states. Tendonitis technically refers to active inflammation of the tendon, while tendinopathy is the broader term covering both inflammation and the longer-term structural changes that develop when a tendon is repeatedly overloaded without full recovery. In practice, both point to the same underlying question: what has been loading the tendon beyond its capacity, and where is that load actually coming from.
